Job summary

Angels Care Hospice in Nebraska is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will work with multidisciplinary teams to deliver professional in-home care and execute admission visits to determine eligibility for home health services.

Responsibilities include developing the plan of care and collaborating with families, while benefiting from competitive salary plus mileage, on-call pay, generous PTO, and comprehensive health benefits.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver's License.
• One-year's experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient's home care.
• Interact with other medical disciplines and provide professional care.
• Perform admission visit, determine eligibility for home care services, and develop the plan of care.
