Job summary

Angels Care Home Health is seeking an RN Case Manager to coordinate comprehensive home health care for patients in their homes. You will assess needs, collaborate with medical teams, and ensure delivery of high-quality nursing care.

The role includes admission visits, determining eligibility for home care, and developing individualized care plans. Nebraska McCook location with competitive benefits and growth opportunities.

Qualifications

• Graduate of an accredited nursing program (diploma/ADN/BSN).
• Current state RN license.
• Current state Driver's License.
• One year RN experience; two years preferred.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visits and determine eligibility for home care; develop the plan of care.
