Job summary

Coastal Hospice is seeking a dedicated Registered Nurse (RN) to provide quality hospice care in patients' homes and residential facilities. You will collaborate with physicians, social workers, and families to develop and execute individualized care plans, ensuring comfort and dignity at end of life.

Responsibilities include accurate documentation, patient education, and participation in on-call rotations. Must have an RN license, CPR, and be able to travel locally.

Qualifications

• Graduate of an accredited school of nursing.
• Two (2) years of RN experience in a clinical setting; hospice preferred.
• Current valid RN license; CPR certification required; valid driver’s license and auto liability insurance; not listed in OIG Excluded Provider listing.

Responsibilities

• Administer hospice care in patients' homes or facilities.
• Coordinate care with interdisciplinary team and families.
• Develop and implement written Plan of Care per physician orders.
• Document patient status timely and accurately.
• Educate patients and families on care plans and symptom management.
• Participate in on-call rotation as required.
