Job summary

Infinity Hospice Care LLC is seeking a dedicated RN to administer nursing care in clients’ homes across Texas. You will coordinate with interdisciplinary teams and families, ensuring high-quality home health and hospice services.

A valid RN license and strong clinical judgment are essential. Responsibilities include comprehensive assessments, care planning, and on-going care reevaluation, with emphasis on safety, infection control, and regulatory compliance.

Qualifications

• One (1) year experience as an RN in a clinical care setting.
• Preferred, Home health experience.

Responsibilities

• Promote /exemplify the Company's mission, vision, and values.
• Complete initial and ongoing assessments to identify patients' needs and plan care (includes OASIS).
• Regularly reevaluate nursing needs and outcomes; assist recovery or maintenance of life functions.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Provide effective communication to clients, families, and health care professionals; participate in Case Conferences.
