Ennoble Care is hiring a full‑time Hospice RN Case Manager serving patients in the Philadelphia area. You will conduct skilled nursing assessments, planning, and care coordination under leadership while traveling to patients’ homes to maximize comfort for patients and families.

Responsibilities

• Manage assigned patient caseload within your territory across all phases of the nursing process.
• Document notes in the EMR accurately and timely.
• Coordinate with attending physicians, hospice staff, and other agencies to ensure optimal patient care.
• Educate and coach families on levels of care and what to expect.
• Participate in care conferences and contribute to team discussions.
