Job summary

Beacon Home Health is seeking an RN Case Manager to join our Home Health team in Goodlettsville, TN. This role focuses on managing and delivering the full plan of care for each patient while evaluating performance to improve care quality and productivity.

The position requires at least one year of RN experience, active RN license in the state, and a valid CPR certification. A dependable vehicle and ability to travel within the territory are essential.

Qualifications

• One year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Create, implement, execute, and coordinate the plan of care to promote wellness and prevent re-hospitalization for home health patients.
• Perform initial and comprehensive nursing evaluations to determine care focus and develop plans within state guidelines.
• Maintain accurate documentation including patient condition, goals, interventions and responses.
• Establish and maintain an efficient visit route.
• Provide high-quality clinical services within the scope of nursing practice from admit to discharge.
