Job summary

West Tennessee Healthcare is seeking a Registered Nurse to plan and deliver care for hospice patients in Jackson, TN. You will assess, implement, and evaluate care using the nursing process and collaborate with an interdisciplinary team to maintain comfort and quality of life.

The role requires supervising care provided by licensed and unlicensed staff, participating in on-call rotations, and adhering to professional standards.

Qualifications

• Completion of a diploma, associate, or baccalaureate degree in nursing.
• Baccalaureate degree preferred.

Responsibilities

• Assess home care needs and document in the patient’s medical record.
• Provide direct care to patients per interdisciplinary Plan of Care and manage patient case.
• Collaborate with IDT to update the Plan of Care every 14 days or as needs change.
• Supervise nursing staff and hospice aide care; assign based on acuity.
• Participate in on-call rotation and performance improvement initiatives.
