Job summary

Adoration Health in Chattanooga, TN is seeking a dedicated Hospice RN Case Manager to deliver compassionate, patient-centered care at home. You will assess needs, develop care plans, administer medications, and coordinate with the interdisciplinary team.

This role offers meaningful work, on-call rotation, driving to patient homes, and opportunities for growth within a growing hospice division of Adoration Health.

Qualifications

• Must be a graduate of an accredited nursing program with active TN RN license.
• Minimum 1 year of nursing experience; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of the hospice philosophy and patient-centered care.
• Valid driver’s license with insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs of patients.
• Create and implement hospice care plans aligned with patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.
• Collaborate with interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by local branch.
