Job summary

Autumn Hospice is hiring an RN Case Manager to coordinate and deliver compassionate hospice care. You will assess patients and families, develop individualized plans, and collaborate with an interdisciplinary team to ensure continuous care and support at end-of-life stages.

Qualifications

• Associate degree in nursing is required.
• Current state RN license is required.
• Minimum of two years of professional nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Excellent verbal and written communication skills.

Responsibilities

• Complete an initial assessment of patients and their families to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs.
• Use assessment data to develop and update individualized care plans.
• Collaborate with the interdisciplinary healthcare team, patients, and families to ensure coordinated, continuous care.
• Counsel patients and families regarding end-of-life care and support.
• Initiate appropriate preventive and palliative nursing interventions.
• Communicate with physicians regarding patient needs and report changes in condition.
• Supervise Hospice Aides.
