Pay ranges from $38 to $44 per hour plus on-call pay; full-time benefits include health/dental insurance, a 401(k) with 100% match up to 4%, no vesting period, PTO accrual and flexible schedule.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in state.
• Must be a licensed driver with a vehicle and valid insurance.
• One to two years of acute care and home care experience preferred.

Responsibilities

• Complete initial and ongoing patient/family assessments for hospice needs.
• Develop and revise patient care plans with the inter-disciplinary team.
• Administer medications and treatments per physician orders.
• Educate patients and families on care, safety, and self-care.
• Act as Case Manager coordinating care for assigned caseload.
• Participate in on‑call duties and ensure equipment readiness.
• Maintain accurate clinical notes and communicate changes to the team.
