Job summary

Absolute Hospice is seeking an experienced, compassionate RN to join our hospice team as a Hospice RN Case Manager. This full-time role involves direct patient care, coordinating an interdisciplinary team, and managing patient records to ensure quality hospice care. A $2,500 sign-on bonus is offered.

The RN will assess patients, develop and implement care plans, and educate patients and families on care practices, medications, and end-of-life considerations within a supportive team environment.

Qualifications

• Must hold RN licensure in the state of practice.
• Experience in hospice care is preferred.
• Strong assessment and care planning skills.

Responsibilities

• Ensure high quality of care documented per the Plan of Care.
• Perform initial and ongoing patient assessments.
• Develop and implement hospice plans of care with the care team.
• Educate patients and families on medications, dressing changes, oxygen, and hospice philosophy.
