Job summary

Interim HealthCare Inc. is seeking a Hospice Registered Nurse Case Manager (RN). You will be called to care when you're needed most, supporting a full range of patient services to bring comfort and dignity to our clients.

As part of Interim HealthCare, you'll oversee the care plan, collaborate with the care team, and deliver one-on-one patient care in a flexible, full-time role with competitive pay, benefits, and a $5k sign-on bonus. Daily pay option is available.

Qualifications

• ADN or AAS required for RN licensure.
• Current RN license in the state of assignment.
• Current CPR/FA certification.
• Reliable transportation to/from care sites.
• 1 year RN hospice or similar experience preferred.

Responsibilities

• Oversee implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adjust care as needed.
