Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager (RN) to coordinate patient care and manage plans of care in collaboration with the care team. The role emphasizes one-on-one patient care, integrity, and a patient-centric approach across the care continuum.

The ideal candidate holds an ADN or AAS in Nursing (BSN preferred), an active RN license in the applicable state, and CPR/FA certification.

Qualifications

• ADN or AAS in Nursing required; BSN preferred.
• Current RN license in the state where position is based.
• Current CPR/FA certification.
• Reliable transportation to/from care sites and/or work locations.
• One year of professional RN experience in hospice or similar setting preferred.

Responsibilities

• Oversee the patient's plan of care with the care team and physician oversight.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers to adjust care as needs evolve.
