Jobot is seeking a Full-time Hospice RN Case Manager in Queens, NY. The RN will cover a small territory of nearby ZIPs, following a 830 AM–5 PM schedule, Monday through Friday.

This role focuses on end-of-life care, coordinating services, and ensuring comfort for terminally ill patients in home and facility settings. Candidates must hold an active NY RN license with at least 1 year in hospice/palliative care, possess a BSN preferred, and have reliable transportation.

Qualifications

• RN with active NY State RN License in good standing and at least 1 year of RN experience in Hospice/palliative care or Med-Surg/Oncology.
• BSN degree highly preferred.
• RN must have a car.

Responsibilities

• Deliver skilled nursing care in home and facility settings for terminally ill patients.
• Manage caseload of four Hospice patients per day and coordinate hospice services.
• Create and follow the Plan of Care to maximize comfort and quality of life.
• Provide physical and psychosocial assessments to develop Goals of Care.
• Educate patients and families on disease progression, symptoms, and meds.
• Collaborate with physicians and IDT to update care plans.
