Job summary

Jobot is seeking a skilled Hospice Admissions RN for the Nassau County NY and Queens area. This full-time role involves performing initial assessments, developing care plans, and admitting eligible hospice patients to the program, with NHPCO standards followed.

Eligible candidates will have BSN preferred, at least 2 years in Hospice/Oncology/Med-Surg, and will receive a $15,000 new hire bonus, pension and 403B retirement plan, life insurance, generous PTO and comprehensive benefits.

Qualifications

• RN with BSN preferred.
• 2+ years in Hospice, Oncology, Med-Surg, or Palliative Care.
• Ability to perform initial physical and psychosocial assessments and develop care plans.

Responsibilities

• Perform initial assessments of hospice patients.
• Develop individualized plans of care and goals.
• Admit eligible patients to the Hospice program following NHPCO standards; territory Nassau County NY and Queens.
