Alliance for Care at Home in Queens, NY seeks a Full-time Hospice RN Case Manager to deliver skilled nursing care in home and facility settings, with a small territory and 4-week vacation. You will manage a caseload of terminally ill patients, creating care plans focused on comfort and quality of life.

Mon-Fri, 8:30 AM-5 PM, with coordinated home-based hospice services and family education. The role requires NY RN licensure and hospice or related experience; BSN preferred; car is required.

Qualifications

• RN with active NY State RN License in good standing and at least 1 year RN experience in Hospice or palliative care, or Med Surg or Oncology.
• BSN degree highly preferred.
• RN MUST have a car.

Responsabilités

• See four Hospice patients per day and manage a caseload of terminally ill patients in home and facility settings.
• Create and follow plan of care to maximize patient comfort and quality of life.
• Provide physical and psychosocial health assessments for Hospice Plan of Care and Goals of Care.
• Educate patients and families on disease progression, symptom management, and medication administration.
• Collaborate with physicians to titrate medications and coordinate with the IDT team.
• Assess and recommend equipment and supplies; coordinate delivery and maintain proper inventory at home.
• Maintain accurate and timely clinical documentation.

Driver's license / car required
