Job summary

Jobot is seeking a Field-based Hospice RN Case Manager to join our NYC metro healthcare team. You will cover a small Bronx territory, visiting terminally ill patients in home and facility settings, delivering skilled nursing care and coordinating hospice services.

You will create and follow plans of care, educate families, collaborate with physicians and IDT, and maintain accurate clinical documentation. A current NY RN license and car are required; BSN preferred.

Qualifications

• Active NY State RN license in good standing.
• At least 1 year RN experience in hospice/palliative care or Med/Surg/Oncology.
• BSN degree is preferred.
• Must have a car.

Responsibilities

• Provide field-based hospice care to terminally ill patients in home and facility settings.
• Manage a small Bronx territory with 3 zones and coordinate care plans.
• Develop, implement and update Hospice Plan of Care and Goals of Care.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate and timely clinical documentation.
