Jobot is seeking a Hospice RN Case Manager in SOUTH Brooklyn, NY, offering a $15,000 sign-on bonus and a comprehensive benefits package. The role involves a small territory, four hospice visits daily, and coordination of care for end-of-life patients.

Qualified applicants are expected to be an RN with NY license and preferably a BSN, with a car for local travel. Schedule is 830 AM–5 PM, Monday–Friday, in-home and community-based settings.

Qualifications

• RN license active in NY in good standing; hospice/palliative care or Med-Surg/Oncology experience preferred.
• BSN degree highly preferred.
• RN must have a car for territory visits.

Responsibilities

• Conduct home visits to terminally ill patients; four visits per day.
• Create and follow plan of care to maximize comfort and quality of life.
• Coordinate hospice services and communicate with IDT to develop patient-centered plans.
• Educate patients and families on disease progression, symptom management, and medications.
• Maintain accurate clinical documentation and ensure timely charting.
