Qualifications

• ADN or AAS in Nursing required.
• BSN preferred.
• Current RN license to practice in the state of the position.
• Current CPR/FA certification.
• Reliable transportation to/from care sites and/or work locations.
• One year of RN hospice or similar setting experience preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers as care evolves.
