Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager (RN) to be called to care when needed, delivering compassionate patient care and coordinating services to bring comfort and dignity to clients.

You will oversee care plans, collaborate with the care team, and ensure accurate evaluation of patient needs while maintaining a patient-centric approach. Full-time hours with the benefit of flexible scheduling and daily pay are offered.

Qualifications

• ADN or Associate of Applied Science in Nursing (AAS) required; BSN preferred.
• Current RN license in the applicable state.
• CPR/FA certification required.

Responsibilities

• Oversee implementation and ongoing assessment of the patient\'s plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adapt care.
