Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager (RN) to coordinate and deliver compassionate, patient-centered care. You will work with an interdisciplinary team to assess and implement the patient’s plan of care, ensuring comfort and dignity for clients at all times.

The role emphasizes flexible scheduling with full-time commitments, competitive pay and benefits, and a daily pay option. Prior hospice RN experience is preferred.

Qualifications

• Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required.
• Bachelor of Science in Nursing (BSN) preferred.
• Current license to practice as a Registered Nurse (RN) in the state of employment.
• Current CPR/FA certification.

Responsibilities

• Oversee the patient's plan of care with the care team including spiritual care coordinator and medical director.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and coordinate with providers as needs evolve.
