Devoted Care Hospice is seeking a compassionate, experienced Registered Nurse Case Manager to join our growing clinical team in a full-time role. You will oversee the nursing care and coordination of an assigned patient caseload, serving as a primary clinical resource for patients, families, and the interdisciplinary team.

Responsibilities include comprehensive nursing assessments, plan of care development, condition monitoring, and timely clinical documentation.

Qualifications

• Current Texas RN license in good standing.
• Minimum one year of RN experience; hospice experience preferred.
• Strong clinical assessment and independent decision-making.
• Experience coordinating care with an interdisciplinary team.

Responsibilities

• Manage an assigned hospice patient caseload and provide ongoing clinical oversight.
• Complete comprehensive nursing assessments and routine/PRN visits.
• Develop and update individualized plans of care.
• Assess changes in condition, disease progression, and symptom management needs.
• Manage pain and end-of-life symptoms per hospice best practices.
• Coordinate care with aides, social workers, chaplains, physicians, facilities, and pharmacies.
• Document accurately and timely in the EMR; participate in team meetings.
