Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager (RN) to provide patient-centered care in Morgan/Perry County and beyond. You will lead the patient’s plan of care, coordinate with the team, and ensure a compassionate approach that supports dignity and comfort for clients.

Requirements include an ADN/AAS (BSN preferred), current RN license in the state, CPR/FA certification, reliable transportation, and 1 year of hospice or similar experience

Qualifications

ADN or AAS in Nursing required; BSN preferred.
Current RN license in the state of practice.
Current CPR/FA certification.
Reliable transportation to care sites.
1 year RN hospice or similar experience preferred.

Responsibilities

Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
Develop and implement a comprehensive care plan addressing patient needs.
Communicate patient conditions and coordinate with providers as needs evolve.
