Job summary

Interim HealthCare Hospice & Palliative Care in Ohio is seeking a Hospice Registered Nurse Case Manager (RN) to coordinate patient care, assess needs, and work with the interdisciplinary team to deliver comfort and dignity. You will oversee the patient’s plan of care and collaborate with the team to adapt as needs evolve.

The role requires ADN or AAS, RN licensure in the state, CPR/FA certification, reliable transportation, and 1 year of hospice or similar experience is preferred.

Qualifications

• Education: ADN or AAS required; BSN preferred.
• Licensure: Current RN license in Ohio.
• CPR/FA certification required.
• Reliable transportation to care sites.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adapt care as needs evolve.
