Job summary

Interim HealthCare Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager to provide compassionate, patient-centered care across the full range of services. The role emphasizes one-on-one patient care, collaboration with the care team, and timely communication with physicians to adjust plans of care as patient needs evolve.

Qualified candidates will hold an RN license, have CPR/FA certification, and preferably possess hospice experience with an ADN/AAS or BSN degree.

Qualifications

• RN license in the state where the position is located (active)
• Current CPR/FA certification
• 1 year RN hospice or similar care experience preferred

Responsibilities

• Oversee implementation and ongoing assessment of the patient’s plan of care with the care team
• Develop and implement a comprehensive care plan addressing patient needs
• Communicate patient conditions and coordinate with providers as needs evolve

Education

Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS)

Bachelor of Science in Nursing (BSN)
