Job summary

Adoration Health is hiring a Hospice RN Case Manager to provide high-quality, patient-centered end-of-life care in the Indianapolis area. You will assess needs, create care plans, deliver direct nursing care, manage pain and symptoms, and educate families.

This role requires an active RN license and CPR, with 1 year of nursing experience and hospice exposure preferred. Flexible scheduling, strong team support, and career growth opportunities are offered.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle in good order and CPR current.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team; maintain timely documentation.
• Participate in on-call rotation as required.
