Job summary

Interim HealthCare Inc. is seeking a Hospice Registered Nurse Case Manager (RN) to provide compassionate, patient-centered care. You will oversee care plans with the interdisciplinary team and ensure evaluation and updates to address evolving needs in Morgan/Perry County and east toward Cambridge.

Qualifications

• Education: ADN or AAS required; BSN preferred.
• Licensure: Current RN license in the state for this position.
• CPR/FA certification current.
• Reliable transportation to/from care sites and/or work locations.
• One year of professional RN experience in hospice or similar setting preferred.

Responsibilities

• Oversee the implementation and ongoing assessment of the patient's plan of care with the care team.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers to adjust care as needed.

Education

Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required

Bachelor of Science in Nursing (BSN) preferred
