Job summary

Interim HealthCare Inc. is seeking a Hospice Registered Nurse Case Manager (RN) to provide compassionate patient care across Morgan/Perry County and east toward Cambridge. You will oversee care plans and collaborate with the care team to ensure quality, patient-centered support.

Qualifications

• ADN or AAS required; BSN preferred
• RN license in the state of placement
• Current CPR/FA certification
• Reliable transportation to care sites/work locations
• 1 year RN experience in hospice or similar setting preferred

Responsibilities

• Oversee implementation and ongoing assessment of patient plan of care with the care team
• Develop and implement a comprehensive care plan addressing patient needs
• Communicate patient conditions and collaborate with providers as needs evolve
