VIA Health Partners is a top-nonprofit hospice and palliative care provider actively serving Fayetteville, NC. We are seeking an experienced Registered Nurse to coordinate and deliver comprehensive patient and caregiver care in a local setting.

The role emphasizes patient education, collaboration with the IDG, documentation, and adherence to infection control standards, with opportunities for CHPN certification and career growth within a community-focused organization.

Qualifications

• A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• Current, active, and unrestricted RN license required.
• Multistate nurse license (NLC) rules apply; compact state practice allowed.
• CPR certification required; CHPN preferred.
• Minimum 1 year inpatient experience; 2 years for other nursing roles; hospice/palliative care valued.
• Computer proficiency required.

Responsabilités

• Delivery of patient care and coordination of care for patients and caregivers.
• Conduct nursing assessments to manage patient and caregiver needs.
• Develop, implement, and update care plans to ensure continuous, goal-oriented care.
• Facilitate patient and caregiver independence where possible.
• Follow VIA Health Partners protocols, orders, and guidelines in hospice care.
• Provide education to patients and families; answer questions and support learning.
• Communicate effectively with the IDG, patients, caregivers, and other health professionals.
• Maintain timely and accurate clinical documentation per guidelines.
• Attend IDG conferences; maintain strong teamwork and professional relationships.
• Comply with infection control and universal precautions; safe handling of chemicals.
