Job summary

Hospice of Western & Central Massachusetts is seeking a compassionate and experienced Hospice RN Case Manager to join our team. The role focuses on delivering comprehensive hospice care to patients and their families, with a strong emphasis on building relationships and coordinating services in home and community settings.

The position is on-the-road, Monday through Friday 8a-4:30p, with mileage reimbursement and pay up to $49 per hour, paid weekly.

Qualifications

• 1-2 years of acute care experience in an institutional setting and 1-2 years of home care experience.
• CPR certification is required.
• Active and unencumbered RN license in Massachusetts.
• Bachelor's degree in Nursing; hospice care experience preferred.

Responsibilities

• Plan, organize, and direct hospice care for patients and families in their homes and communities.
• Conduct initial, comprehensive, and ongoing assessments to determine patient and family needs.
• Develop and implement patient care plans, collaborating with patients, families, and healthcare providers.
• Provide professional nursing care, utilizing elements of the nursing process.
• Supervise hospice aides and participate in revising patient care plans as necessary.
• Collaborate with healthcare providers to ensure patient continuity of care.
• Provide education and guidance to patients and families on disease management, wound care, and symptom management.
• Participate in patient care conferences to review patient progress and make adjustments to care plans.
