LiveWell Homecare Agency, LLC is seeking an experienced RN Case Manager to provide compassionate end-of-life care in patients' homes around Bloomington and nearby areas. You will assess patients, coordinate plans, and collaborate with physicians and care teams.

The role emphasizes strong clinical judgment, independent work, and clear communication with families and partners to ensure comfort and quality of life in home hospice settings.

Qualifications

• Active RN license in the state of practice (IL).
• Graduate of an accredited Nursing program.
• Preferably 1+ year hospice, home health, or related experience.
• Strong clinical judgment and ability to work independently.
• Excellent communication skills with care teams and families.

Responsibilities

• Conduct comprehensive, ongoing patient assessments to determine evolving needs.
• Develop and adjust personalized care plans in collaboration with physicians and care teams.
• Administer medications and treatments per physician orders.
• Maintain accurate and timely clinical documentation.
• Serve as a communication link among families, physicians, and community health partners.
• Educate patients and caregivers to foster confidence and understanding.
• Participate in on-call rotations as per team policies.
