Job summary

St. Croix Hospice seeks a Registered Nurse Case Manager to plan, organize and direct hospice care in home settings across Indiana. You will assess patients and families, develop care plans, and coordinate with the interdisciplinary team to deliver compassionate, evidence-based nursing services.

The role requires an Indiana RN license and 1–2 years of acute care experience, with home care preferred. Strong communication and patient/family education are essential for quality outcomes.

Qualifications

• Graduate of an accredited school of nursing.
• Current Indiana RN license.
• 1–2 years recent acute care experience; home care preferred.
• Excellent observation, verbal and written communication skills.
• Must have a licensed driver and insured vehicle.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises plans of care with patient and family.
• Administers medications and treatments per physician's orders.
• Educates patients and families on self-care and care plans.
• Coordinates patient care with the hospice interdisciplinary team.
• Participates in on-call duties and ensures resources are available.
• Maintains accurate clinical notes and reports changes to physicians.
