Job summary

Advocate Aurora Health is seeking a full-time Hospice RN to coordinate patient and family care for terminally ill patients across multiple settings. You will educate families on disease trajectories, collaborate with physicians and the IDG, and ensure the plan of care reflects patient needs and compliance with regulations.

The role requires current RN licensure, BLS certification, and 1–2 years of nursing experience in home health or hospice.

Qualifications

• RN license required in the practicing state.
• BLS for Healthcare Providers certification required or obtainable on hire.
• 1–2 years nursing experience in home health, hospice, or related settings.

Responsibilities

• Coordinate patient/family care for terminally ill patients in hospice settings.
• Education of patients and families on disease trajectory and hospice care.
• Review caseloads to ensure appropriate hospice care and referrals to IDG members.
• Administer medications and treatments as prescribed.
• Lead the interdisciplinary IDG and develop the Plan of Care with the team.
