Job summary

Your Health Organization is seeking a Registered Nurse to join our growing team as a Nurse Case Manager serving Georgia and South Carolina facilities, homes, and clinics. The role requires travel to patients' homes or ALFs/ILFs and coordination with care teams to initiate telemedicine and care plans.

Ideal candidates have RN licensure in good standing, 3+ years of clinical experience, strong communication skills, and the ability to work flexible hours across a wide service area.

Qualifications

• Must be a Registered nurse with license in good standing.
• Minimum 3 years clinical experience preferred.
• Experience in community settings preferred.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Ability to manage multiple responsibilities and travel.

Responsibilities

• Facilitate receiving medical records from primary providers and specialists.
• Review medical records and complete consents with patients.
• Enroll patients in Care Management if eligible and initiate a Care Plan of Care.
• Capture diagnoses at highest specificity and document Care Management activities.
• Coordinate with care team and telehealth visits when necessary.
• Coordinate with home health, hospice, and other providers as needed.
• Ensure safe home environment and evaluate need for RPM devices or DME.
