Job summary

Rocky Mountain Care Hospice is seeking a compassionate Hospice Registered Nurse (RN) to provide skilled, patient-centered care throughout the Davis/Weber area. You will complete hospice nursing visits, assess patients, and educate families while coordinating care with the interdisciplinary team.

Qualifications

• Must have a current, unencumbered RN license.
• Certification in CPR.

Responsibilities

The RN Case Manager consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.Provide all patient care in a manner that reflects respect for patient rights, dignity, and expressed needsAssess the patient’s physical, psychological, and social needsAssess pain and implement appropriate pain management techniquesDocument patient history, symptoms, assessments, tests, and care according to guidelinesReview patient history, diagnostics, and lab data, and report any abnormalitiesQUALIFICATIONSMust have a current, unencumbered RN licenseCertification in CPRPrior clinical experienceExcellent communication skillsDemonstrated ability to maintain a high level of professionalism during stressful timesStrong critical thinking skillsAbility to adapt to different clinical settings and environments
