Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Youngstown, OH. This role focuses on coordinating patient care, developing individualized plans of care, and collaborating with physicians and families to ensure comfort, dignity, and quality of life.

As part of a supportive team, you will manage visits, documentation in Homecare Homebase, and after-hours coverage as needed.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Strong assessment and care coordination skills.
• Excellent communication with patients and families.

Responsibilities

• Provide skilled nursing care and symptom management per hospice standards.
• Complete scheduled visits and admissions in Homecare Homebase (HCHB).
