Job summary

Rocky Mountain Care Hospice in Salt Lake City seeks a compassionate Hospice Registered Nurse (RN) to join our team on a 7-on / 7-off schedule, delivering patient-centered end-of-life care in homes and community settings.

You will perform comprehensive assessments, coordinate with an interdisciplinary team, and educate families while maintaining documentation and quality standards in Utah.

Qualifications

• Active Utah RN license.
• Strong assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and with a team.
• Experience in hospice, home health, or related nursing preferred.

Responsibilities

• Complete hospice nursing visits and comprehensive patient assessments.
• Provide skilled nursing and comfort-focused care.
• Monitor changes in condition and respond to patient needs.
• Educate and support patients, families, and caregivers.
• Coordinate care with interdisciplinary hospice team.
• Maintain accurate and timely clinical documentation.
• Make independent clinical decisions within your scope of practice.
