We are seeking a dedicated and experienced Registered Nurse Case Manager to join our Hospice team. The ideal candidate will play a vital role in coordinating patient care, ensuring seamless transitions across various care settings, and optimizing health outcomes. Duties

Conduct comprehensive patient assessments, including vital signs and physiological evaluations, to determine care needs.

Develop and implement individualized care plans in collaboration with multidisciplinary teams, focusing on palliative needs.

Coordinate with hospital discharge planners to ensure continuity of care.

Collaborate with physicians, and other healthcare professionals in managing complex cases involving geriatric and end-of-life care.

Provide ongoing patient/family education on treatment plans, medication management, and hospice care strategies.

Ensure compliance with hospice policies, safety standards, and regulatory requirements including HIPAA and medical terminology standards.

Qualifications

Valid RN license with experience in home care patients.
