The Care Team Hospice in Springfield/Jacksonville is seeking a compassionate Hospice RN Case Manager to coordinate patient care and develop individualized plans of care. You will collaborate with physicians and the interdisciplinary team to ensure comfort, dignity, and quality of life for patients and families.

Active RN license is required; hospice experience is preferred. Homecare Homebase (HCHB) experience is a plus.

Responsabilidades

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in HCHB.
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.
