Job summary

Bristol Hospice is looking for a dedicated RN Case Manager to provide compassionate, whole-person hospice care and guide patients and families through a meaningful journey. You will develop care plans, monitor clinical needs, and coordinate with interdisciplinary teams to ensure dignified end-of-life support.

The role requires an active RN license in the state of employment, CPR certification, a valid driver's license with reliable transportation, and strong written and verbal communication

Qualifications

• Valid RN license in good standing in the state you will work in.
• Valid driver's license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.
• Flexible, team-oriented attitude and willingness to support additional duties as needed.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• And contribute to other supportive duties as part of a compassionate, mission-driven team.
