Job summary

Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and families in Springfield and surrounding communities. The RN will perform nursing assessments, planning, and care to maximize comfort, responding promptly to calls, visiting patients as needed, and handling after-hours admissions and deaths as required.

Ideal candidates have prior hospice, home health, geriatric, or general med/surg nursing experience and a demonstrated commitment to hospice

Qualifications

• Licensed RN in the state where care is provided.
• Experience in hospice, home health, geriatrics, or med/surg nursing.
• Demonstrated commitment to hospice philosophy of care.

Responsibilities

• Receives patient assignments from the Patient Care Manager or Case Manager and coordinates care with physicians, hospice staff, and agencies.
• Maintains communication with families and attends team meetings and care conferences.
• Performs nursing assessments, planning, and care to maximize comfort and health of patients and families.
• Instructs patients and caregivers on medication administration, personal care, and safety procedures.
• Maintains up-to-date patient records and submits required Medicare and hospice paperwork.
• Provides 24/7 coverage and supports the care team as needed.
• Supports bereavement and family communication as part of care.
• Contributes to pain management and ongoing hospice education.
