Job summary

• Maintain up-to-date patient records and Medicare documentation.
• Instruct patients and caregivers on meds, self-care, lifting, and safety.
• Provide 24/7 coverage as needed and participate in care conferences.
• Support families during bereavement and end-of-life care.
