Qualifications

• Registered Nurse licensed in the state where care is provided.
• Hospice, home health or geriatrics experience preferred.

Responsibilities

• Receives patient assignments and coordinates care with physicians and hospice staff.
• Maintains clear communication with patient families and attends care plan meetings.
• Documents plans of care and updates patient records for Medicare and Hospice policies.
• Provides education to patients and caregivers on medications, personal care, and safety.
