Job summary

Good Shepherd Hospice in Independence, MO seeks a compassionate Registered Nurse Case Manager to assess, plan and deliver hospice care for patients and families in Independence and surrounding communities.

The RN coordinates with physicians and the hospice team, educates families on medications and care, and maintains accurate records while providing after-hours support as needed. This role requires hospice or geriatrics experience and a demonstrated commitment to compassionate end‑of‑life care.

Qualifications

• Registered Nurse licensed in the state where care is provided.
• Experience in hospice, home health, geriatrics or general medical/surgical nursing.
• Demonstrated commitment to the hospice philosophy of care.

Responsibilities

• Receives patient assignments and initiates coordination with attending physicians, hospice staff and other agencies to optimize care.
• Cultivates communication and bereavement skills.
• Maintains regular communication with the attending physician regarding patient/family care.
• Informs Patient Care Manager of unusual or potentially problematic issues.
• Attends Hospice team meetings and care plan conferences as needed.
• Provides support at time of death and ensures 24/7 coverage for patients and families.
