Job summary

Heart to Heart Hospice in Pearsall, TX is seeking an RN Case Manager to deliver compassionate hospice care to patients and families. This full-time role requires clinical assessment, care planning, and coordination with the care team to support quality of life at end of life.

You will perform ongoing assessments, implement and revise the care plan, collaborate with physicians and IDG meetings, provide on-call coverage, and ensure timely documentation while complying with laws and policies.

Qualifications

• RN license in good standing.
• Two years of RN experience preferred in healthcare/clinical setting.
• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients to identify physical, psychosocial, environmental needs and ADLs.
• Implement the individualized plan of care (POC) and revise as necessary.
• Collaborate with patient/caregiver, physicians and other health professionals to coordinate care.
