Job summary

Hospice of North Carolina seeks a dedicated Registered Nurse Case Manager (RNCM) to deliver compassionate, patient‑centered hospice care across our community. You’ll coordinate services, support patients and families, and collaborate with an interdisciplinary team to ensure comfort and dignity.

Ideal candidates hold an active RN license, have at least 1 year in medical‑surgical/acute care, and are proficient with electronic medical records.

Qualifications

• Active RN license currently in good standing.
• Minimum 1 year of RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in EMR and computer programs; CPR certification may be required by state.

Responsibilities

• Respond to clinical referral information with care and expertise.
• Partner with Clinical Director to assess client eligibility for hospice services.
• As an RN Case Manager, coordinate care and support patients and families.
• Deliver nursing care rooted in proven principles and techniques.
• Educate patients and families to participate in personalized care plans.
• Share call duties as part of the team.
