This role offers a salary range of $60,200 to $107,400 annually, with a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Perform initial nursing evaluation for hospice eligibility.
• Identify patient/family needs and re-assess every 15 days.
• Initiate and coordinate the plan of care.
• Document patient progress and response to care.
• Collaborate with care team to provide support to patients and families.
