Qualifications

Responsibilities
Perform initial nursing evaluation for hospice eligibility.
Identify patient/family needs and re-assess every 15 days.
Initiate and coordinate the plan of care.
Document patient progress and response to care.
Collaborate with care team to provide support to patients and families.
