Gentiva Hospice is seeking a dedicated Registered Nurse to serve as a Hospice RN / Case Manager. The role involves in-home assessments, symptom management, and coordinating with an interdisciplinary team to deliver compassionate end-of-life care.

The candidate will educate patients and families, document care plans, and ensure compliance while supporting caregivers through education and bereavement guidance.

Qualifications

• Current license to practice nursing in the state of employment
• Hospice or palliative care experience preferred
• CPR Certification

Responsabilités

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and provide education to patients, family members and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease process, self-care techniques, end-of-life care, medications, nutrition and dietary needs.
• Communicate changes in condition to appropriate provider and intervene as needed.
• Support continuity of care by coordinating referrals and transitions between care levels.
• Document timely clinical records and ensure regulatory compliance.
• Maintain relationships with IDG and involve team as needed for outcome achievement.
• Participate in on-call and patient support activities as applicable.
