Job summary

New Day Hospice is seeking an experienced Registered Nurse Case Manager to provide nursing care to patients in their homes and coordinate with the Interdisciplinary Group/Team, families, and referring agencies. Responsibilities include comprehensive assessments, care planning, and family education about disease progression and end-of-life care.

The role requires reliability, strong nursing skills, and the ability to travel locally, with on-call responsibilities as assigned.

Qualifications

• Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current RN license in the state and a valid driver's license.
• Two years' RN experience in a clinical setting; hospice experience preferred.
• Nursing skills per standards; good interpersonal skills; palliation/end-of-life skills preferred.
• Must read, write and comprehend English.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patients' needs and family dynamics.
• Develop, document, and revise the plan of care to ensure quality and continuity.
• Educate patients and families about disease process, self-care, end-of-life care, nutrition, and dietary needs; provide training as needed.
