Job summary

Acacia Health is seeking an experienced hospice nurse to plan, coordinate, and deliver home-based care. You will assess patients, develop and revise care plans, and work closely with physicians to meet patient and family needs in home and community settings.

Responsibilities include documenting in HomecareHomebase, educating patients and families, and serving as a case manager to ensure seamless care delivery across interdisciplinary teams.

Qualifications

• CA RN License required.
• CPR certification required.
• CA Driver License required.
• Hospice nursing experience: 1 year (Required).

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patients.
• Initiates the plan of care and revisions as needs change.
• Develops a care plan based on nursing diagnosis and palliative actions.
• Assists patient with activities of daily living and supports self-sufficiency.
• Acts as Case Manager to coordinate patient care.
• Completes and submits clinical notes in HomecareHomebase.
• Communicates with physician regarding patient needs and changes in condition.
• Teaches self-care techniques to patient and family/caregiver.
• Attends interdisciplinary meetings.
• Fulfills assigned case work.
